Military leaders and a major military contractor failed to protect a Green Beret who was electrocuted while showering in his barracks in Iraq, the Defence Department's Inspector-General determined in a report released on Tuesday.
The death last year of Staff Sergeant Ryan Maseth, 24, triggered an investigation and also a review of 17 other electrocution deaths in Iraq.
The Inspector-General says "multiple systems and organisations" failed and exposed Maseth to "unacceptable risk".
