Applications are now open for the 2004 New Zealand Herald Junior Sports Awards. Five awards, which offer an engraved medallion and $1500 training grant, will be made by the judges, who may also highly commend up to three applicants.
Sportsmen and women under 20 years of age on December 31, 2004, can apply.
Applicants must give full details of the performance(s) in about 400 words.
Applications must include name, age, date of birth, address and contact details. All nominations must be verified by a coach/parent/club secretary or headmaster.
Applications must be received by December 31 and should be sent to NZ Herald Junior Sports Awards, PO Box 32, Auckland, faxed to (09) 373-6428 or emailed to terry.maddaford@nzherald.co.nz
