A court of inquiry into the death of HMNZS Canterbury seaman Byron Solomon during a routine exercise has pinpointed design flaws with the multi-role ship.
Able Seaman Solomon, 22, drowned when trapped under a capsized rigid hull inflatable boat after it detached from the Canterbury without warning.
The inquiry found significant irregularities in certification for the RHIB and a fast rescue boat.
It was "highly likely" their operation did not meet minimum Safety of Life at Sea requirements and "most certainly do not meet best practice standards".
