Police will today try to piece together how a "routine operation" went so dramatically wrong.
Commissioner Howard Broad said a full internal inquiry was under way following the death of Sergeant Don Wilkinson and the shooting of his colleague.
Detailed risk assessments had been carried out and it "would appear" procedures had been followed, Mr Broad said.
"There is a standard level of concern because armed back-up was available and the tragedy was that it unfolded so quickly that back-up was not in a position to intervene."
However, it is understood the inquiry will look at why Mr Wilkinson was not wearing protective clothing.
Two men appeared in court after the fatal shooting. A Mangere man, 37, has been charged with Mr Wilkinson's murder, while a Waipa man, 34, has been charged with assaulting him. Further charges are expected in relation to the second policeman, who was shot during the incident.
