It was likely Lower Hutt police left a restricted document in the hands of the Mongrel Mob last year, an independent investigation has concluded.
The Independent Police Conduct Authority (IPCA) released a report yesterday stating police failures equated to a breach of confidentiality and amounted to misconduct, but the person responsible had not been identified.
The investigation was launched after details from a police operation order were broadcast on 3News in March.
