Four South Island police officers could face disciplinary action after they failed to act on three 111 calls from a Nelson woman who feared for her life.
An internal investigation has found one Nelson police officer and three Christchurch officers did not act on the calls as a result of human error, and not an error with the 111 system.
The woman, from Stoke on the outskirts of Nelson, called 111 three times in several hours after her brother allegedly punched her in the face on Sunday night.
Senior Sergeant Grant Andrews told The Nelson Mail several officers had not followed through with procedures after the 111 emergency call centre logged the complaint.
Police were attending another family violence matter and a missing elderly woman with Alzheimer's at the time of the calls, he said.
"Things that should have been picked up haven't been picked up and it has allowed this to happen, and it's not good enough."
He said the incident would possibly be dealt with under the police code of conduct but it was unlikely anyone would lose their job.
