Two nurses have been held to account over failings in their management of a prisoner who later died.
The man was aged in his late 30s and suffered a series of blackouts and seizures in prison in 2011 before being taken to hospital, where he died.
He had suffered a serious brain injury, Health and Disability Commissioner Anthony Hill says, in a report made public today. Following an investigation, Mr Hill found the two nurses breached the code of patients' rights.
He said they breached the code of patients' rights, one of them by failings that included not doing an adequate assessment. The other nurse's faults included making a sub-standard health management plan for the man.
