Falling scaffolding and a dropped hammer are among five "near misses" on the multi-million-dollar fix-up job to Auckland Council headquarters.
Council documents reveal five health and safety incidents were reported in May and June on the high-level job to stop stone cladding slabs from falling off the 31-storey tower.
The incidents include a scaffolding plank falling off the tower onto the roof of the SkyCity Grand hotel - smashing the roof and windows - a falling hammer and a man who slipped on the roof during a heavy storm.
The incidents are declared in a report for a council committee to discuss tomorrow.
Kevin Ramsay, council's general manager of corporate finance and property, said apart from one case the incidents reported have been "minor" and he was confident safety standards are being met on the job.
