Four police officers may face disciplinary action after they failed to act on three 111 calls from a woman who feared for her life.
An internal investigation has found one Nelson police officer and three from Christchurch did not act on the calls as a result of human error, and not an error with the 111 system.
The woman from Stoke, on the outskirts of Nelson, called 111 three times in several hours on Sunday night saying her brother had punched her in the face.
Senior Sergeant Grant Andrews told the Nelson Mail police had been attending another family violence matter and searching for a missing elderly woman at the time.
He said the incident could be dealt with under the police code of conduct but it was unlikely anyone would lose their job.
