Rusty wires caused a lifeboat to fall from its sling on a cargo ship in Wellington Harbour, injuring a crewman.
The lifeboat dropped several metres into the water when four wire ropes snapped during a launch and retrieval drill on the cargo ship Da Dan Xia in April 2014.
One crewman on board the lifeboat had removed his safety restraint and was thrown clear of his seat, and sustained a cut to the head.
A report by the Transport Accident Investigation Commission (TAIC) said all four wire ropes being used to raise the lifeboat back on board were severely corroded.
The damage to the ropes was not visible, as they had been coated in a clear plastic sheath intended to protect the lifeboat from damage.
