A Ministry of Health report this year outlined "unacceptably poor clinical judgment and practice by some medical and nursing staff". The ministry investigated several cases, including a homicide allegedly committed by adischarged patient, and four suicides.
They included the death of 26-year-old Shane Fisher in 2006.
His mother, Sally, has collected more than 1000 signatures for a petition calling for a national review of mental health services. The petition will be presented to Coromandel MP Sandra Goudie this week.
