Inquest findings released yesterday into the death of a brain-damaged Nelson baby in 2004 represent the final chapter in her short but troubled life.
In a high-profile trial, her father was charged with murdering her but was acquitted.
Coroner Ian Smith ruled the baby died as a result of asphyxia caused by the deliberate obstruction of her mouth and nose by her father, The Dominion Post reported.
Her family's name remains suppressed.
Mr Smith had concerns that the little girl's serious health defects appeared to have taken a long time after birth to be identified, particularly regarding the involvement of Plunket nurses, social workers, and paediatricians and ophthalmologist from Nelson-Marlborough Health Board.
The family complained to the health and disability commissioner that their shock and trauma was not recognised after the diagnosis and that follow-up care was negligent.
The investigation concluded the actions of Nelson-Marlborough and Canterbury district health boards were appropriate and reasonable.
Mr Smith made no recommendation, saying: "It is clear that in general these concerns have been subsequently addressed or answered via the health and disability commissioner's report."
