Following triage, the doctor said he read the documentation "too quickly" and was too focused on the fact that she might have a head injury and did not pick up that there could be facial injury.
He recorded that his impression was syncope secondary to viral illness and dehydration.
The woman was discharged after an overnight observation, and the doctor did not discuss her case with a senior medical officer prior, as was the requirement.
The woman returned to the hospital that evening after she felt unwell and thought that something was wrong with her face.
She spoke to a ADHB staff members at the front desk and Emergency Department but no triage was completed. Except for a medical certificate issued by a medical officer, there was also no other documentation of this visit.
The woman was subsequently diagnosed with facial fractures, which were corrected by a maxillofacial surgeon.
In his report, Hill made adverse comment about the care provided by the senior house officer, referred to as Dr C, including that he did not pick up on possible facial injury on the woman referred to as Ms A.
"I am critical that Dr C missed a number of opportunities to elicit that Ms A had suffered a facial injury," Hill said in the report.
"I am critical that Dr C did not cover focal tenderness, and that he did not suggest follow-up or consider splinting for Ms A's thumb."
Hill said he was also concerned that the doctor did not discuss the patient's presentation with a senior medical officer before discharging her.
"This requirement is an important safeguard for patients who are being reviewed by junior doctors," he said.
Criticism was also made about the absence of a record of her visit except for a medical certificate and that the board was not able to identify the staff member who spoke to the woman.
Hill also recommended that the board report to the commission on the implementation of its mentoring programme for junior staff and provide evidence of the training sessions provided to triage and clerical staff on the triage process.
The board is also being asked to review its Emergency Department triage process.
