Taylor Maddison says her son is still in shock after a worker at his childcare centre jammed his big toe in a door, leaving it "50 per cent detached".
The Logan mum posted a warning to Facebook telling other parents of the incident that happened at her former South Maclean childcare centre in Queensland earlier this month, news.com.au reports.
Ms Maddison said she got a call from the centre telling her that her two-year-old's toe had been shut in the door but that his toenail was simply lifted and there was a "small cut".
She sent her sister to pick him up, who she said then realised the severity of the incident, telling her he was "in and out of conscious, falling asleep and uncontrollably shaking from shock".
After rushing from work to join them, she said after pulling away the baby wipes the centre had used to control the blood, she realised his toe was 50 per cent detached.
"We immediately dialled 000 for assistance for an ambulance from home," she said.
Ms Maddison said they were sent straight to hospital where her son underwent surgery and skin repair.
She said doctors suspect he might have nerve damage.
"My concern is the childcare centre never provided my son with proper first aid assistance and never contacted an ambulance from the centre directly," she wrote.
"My two-year-old son is still in shock and emotional distress over the incident and cries every time we have to drive past the old kindy and screams he never wants to go back.
"He also cries daily that he never wants to have this happen to his toe again. He cannot play with his brother or his family, can hardly walk around, let alone run like a normal two-year-old should.
"If you are looking for a centre for your children please do your research and ask for others within the community reviews also!"
Ms Maddison's post has attracted thousands of shares and comments.
"That picture of his toe is just frightening," wrote one woman.
"How absolutely terrified your little man must have been!!! No wonder he doesn't want to go back there. I hope the memories of this terrible day for him (and you too, of course), don't linger."
The childcare centre is owned by Gold Coast company G8 Education.
In a statement provided to the Gold Coast Bulletin, a G8 Education spokesperson confirmed an incident occurred on September 12 where a "toddler's toe was accidentally jammed in a storeroom door".
