The Australian man, in his 20s, was trapped in the door, while the Oceanic Discoverer was moored in Napier today, St John Ambulance Hawke's Bay district manager Steven Smith said.
Crew members freed and resuscitated him before emergency services arrived.
Paramedics then treated him at the scene and took him to Hawke's Bay Hospital.
He was in a critical condition.
A witness told NZPA the man was working on the Oceanic Discoverer - a small 63m cruise ship with a maximum capacity of 72 guests.
The company's website says the ship is on a 12-day cruise of New Zealand's coastline, starting in Auckland and ending at Milford Sound.
