Police attended the crash about 4.30pm yesterday.
Police said a Nissan vehicle travelling west towards Matamata crossed the centre line on a moderate left hand bend colliding head on with a Subaru and a Honda car travelling in the opposite direction.
A rear seat passenger in the Subaru died at the scene and the four other occupants were taken to Waikato Hospital by ambulance with serious injuries, police said.
Police enquiries are continuing and Crash Investigators will complete a scene survey and analysis.
Police thank those members of the public who stopped to help at the scene.
